Location: Pekin, IL - Specialty: Certified Nurse Midwife
The Certified Nurse Midwife (CNM) reports to and is under the guidance of the Director of OB/GYN Services, Site Medical Directors, and collaborating physicians, in accordance with the FQHC mission, strategic goals, federal and state laws and regulations, performance and outcome objectives, and accreditation standards. The CNM provides care to women in all stages of pregnancy, childbirth, post-delivery, and other general women’s healthcare.
Essential Functions
- Assesses the health status of patients by obtaining a relevant medical and health history and performing a physical exam based on age and history to identify any medical diseases or disorders and/or needs of the patient.
- Establishes priorities and devises a mutually acceptable plan of care to maximize the health potential of the individual by ordering appropriate diagnostic tests, identifying appropriate pharmacological and non-pharmacological interventions, and developing a client education plan, according to protocols established by HHS.
- Conducts and interprets relevant diagnostic tests, prescribes non-pharmacologic therapies, provides relevant patient education to the individual and/or family, and makes referrals to other appropriate health professionals and community agencies.
- Maintains a process for systematic follow-up by determining the effectiveness of the plan of care through documentation of client outcomes; reassessing and modifying the care plan as necessary to achieve medical and health goals; and maintaining accurate, legible, and confidential records.
- Participates in periodic quality assurance review, including systematic review of records and treatment plans.
- Functions as a role model and preceptor for nurse midwife students completing a formal education program.
- Participates as a team member in the provision of medical and health care, interfacing with professional colleagues to provide comprehensive care for women.
- Supports and participates in HHS’s continuous quality improvement efforts designed to increase patient outcomes; increase patient satisfaction, and improve the utilization of HHS’s human, capital, and physical resources.
- Participates in HHS’s peer review program.
- Participates in HHS collaborative projects as appropriate.
- Updates patients’ records and changes in health status regularly, as clinically indicated.
- Maintains current knowledge by attending educational programs and maintains certification in compliance with current state law.
- Develops and maintains a positive working relationship with patients, department personnel, and outside agencies.
- Supports clinic compliance with all applicable federal, state, local, and HHS rules, regulations, protocols, and procedures governing the clinical provision of medical services as well as those relating to, but not limited to, workplace safety, public health, and confidentiality.
- Works in consultation with clinical teams, direct clinical support staff, and indirect clinical support staff to develop and implement policies and procedures that maximize patient-centered communication and services.
- Maintains and assures confidentiality of patient information in accordance with HHS’s policies.
- Reports building/equipment problems through the appropriate channels.
- Performs any clerical duty or department related task as assigned by supervisor in a continuously changing medical practice.
- Attends all staff meetings, department meetings, and any other meetings as required.

This Community Health Center provides quality, cost-effective healthcare to minority communities, the uninsured, and underinsured of Pilsen Little Village, including Primary Care, Behavioral Health, Dentistry, OB/GYN and Midwifery, Pediatrics, Pharmacy, and In-House Lab Services.
The CNM will:
- provide care to essentially normal women and newborns, antepartally, intrapartally, postpartally and/or gynecologically
- work within an integrated health care setting which provides for medical consultation, collaborative management and referral
Location: Chicago, IL - Specialty: Certified Nurse Midwife
The Coordinator of Midwifery plans, organizes and directs the activities of the Midwifery Program, assuming full responsibility for supervision, training and evaluation of other provider, program development and evaluation, and inter-department liaison.
In this specific role, the coordinator will be responsible for managing a caseload to assure appropriate clinical coverage, creating and executing a plan to reduce the CNM caseload to a level appropriate for the number of CNMs on staff.
Educated in the two disciplines of nursing and midwifery, possesses evidence of certification according to the requirements of the American Midwifery Certification Board (AMCB). Essential duties and responsibilities include:
- Supports the goals and philosophy of the FQHC
- Selects, orients, directs, evaluates and disciplines other CNM’s
- Plans staffing for coverage of assigned in-patients and out-patients areas
- Assumes 24-hour accountability and responsibility for the service
- Develops, implements, and maintains compliance with policies, procedures and guidelines for the Nurse-Midwifery practice. Directs periodic review of policies, procedures and guidelines
- Maintains compliance with AMCB and state requirements
- Promotes, facilitates and monitors quality of care through chart reviews, participation in state-wide peer review, utilization of quality management principals, monitoring of provider activities and productivity and all other activities.
- Prepares Nurse-midwifery service budget, encourages and monitors compliance according to funding source guidelines.
- Identifies individual CNM needs for professional growth and development and provides opportunities to meet those needs.
- Participates in short and long range planning for the service and medical center;
- Establishes and maintains productive working relationships with other personnel and departments;
- Serves as consultant to outside agencies regarding Nurse-Midwifery practice;
- Represents CNM staff in the affiliated hospital and coordinates CNM services with medicine, nursing, medical records, etc
- Meets regularly with Medical Director, Executive Director and other appropriate staff as part of the management team
- Performs clinical Nurse-Midwifery functions in clinic and hospital
- Maintains Midwifery practice statistics
- Performs other related duties as assigned

This is a multi-site FQHC is providing comprehensive medical, dental and behavioral health services within the Chicagoland area. We are financially and programmatically stable and are continuing to grow. We are Joint Commission Accredited and are PCMH Certified. This position is responsible for the provision of high quality clinical oral health care for the patients of the FQHC.
How You’ll Help Patients: - When a dentist is not present in the facility during treatment, and the public health dental hygienist is working alone, their essential duties and responsibilities include: Providing patient treatment, without a dentist first examining the patient and being present in the facility during treatment:
- On patients who are eligible for Medicaid or who are uninsured and whose household income is not greater than 300% of the federal poverty level.
- In a public health setting: federally qualified health center; a federal, State, or local public health facility; Head Start; a special supplemental nutrition program for Women, Infants, and Children (WIC) facility; a certified school-based health center or school-based oral health program; a prison; or a long-term care facility. Or otherwise indicated by the Illinois Dental Practice Act. Promoting oral health and disease prevention.
Charting of oral conditions, which are subsequently reviewed by the supervising dentist.
Planning and performing preventive procedures, including oral hygiene instruction, nutritional counseling, dental X-rays, dental prophylaxis, fluoride, sealants.
Regularly meeting with the supervising dentist for review and consultation of dental treatment provided under public health supervision. - When a dentist is present in the facility during treatment, the public health dental hygienist will have the same essential duties and responsibilities as a dental hygienist:
- Promoting oral health and disease prevention.
- Charting of oral conditions under the dentist’s supervision.
- Taking of dental radiographs.
- Administration of local anesthetics, if certified to do so.
- Performing preventive procedures, including oral hygiene instruction, nutritional counseling, dental prophylaxis, fluoride, sealants.
Is fully supportive of all quality initiatives.
Is fully receptive to other responsibilities as assigned.
Provides all forms of leadership, supervision, direction, training and role modeling to all supportive staff (eg: Dental Assistant).

Location: Gillespie, IL - Specialty: Dental Hygienist
This is dental work at the practitioner level in the care and treatment of the teeth and gums of patients at the FQHC sites and/or mobile units.
- Employees in this position provide educational, preventative and therapeutic, and intra-oral services.
- Employees perform hygienist duties independently, such as examining teeth and other oral structures, cleaning teeth, applying sealants, and topical fluoride and performing radiological procedures under the collaborative of a General Dentist.
- Employees additionally maintain records of dental treatment.
- Employees may be assisted by support staff in the dental clinic. Employees in this position must work at multiple health centers and/or mobile units and on varying shifts as assigned based on health center needs and must work independent of the collaborating dentist.
- Staff in this position must perform practices of a Public Health Dental Hygienist without authorization, assignment or examinations by a dentist, but under policy, protocol, and collaboration from a General Dentist.

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required:
- Assists the Dental Director in the oral health management of all dental patients.
- Obtains baseline oral hygiene assessment data based upon accepted principles of oral hygiene and periodontal care.
- Records patient-hygienist transactions as they occur in the patient’s dental record so that the dental record accurately and completely reflects the nature of the contact, the condition of the patient and the care or treatment provided.
- Provides oral health education and appropriate individual counseling for all dental patients.
- Provides clinical oral hygiene services including dental prophylaxis, scaling, sealant application and fluoride application consistent with accepted professional practices and standards and in compliance with applicable state law and the center’s clinical protocols.
- Integration of appropriate patient dental hygiene care with other heal care professionals involved in the patient’s health care management.
- Organizes and performs preventive oral health presentations at on-site and off-site locations. Performs independent procedures as delegated and directed by dentists in accordance with state regulations and law as well as Dental Department protocol and directive.
- Assist in the maintenance of all Dental Department areas in compliance with Dental Department directive and policies as well as center policies and procedures relative to infection control, exposure control and safety issues.
- Assists with various clinical and administrative functions of the dental clinic department as appropriate and time permits. Insures the proper disposal of all contaminated or potentially contaminated materials in accordance with Dental Department directive and center policy as well as state federal regulations(s).
- Travels between sites when necessary to meet operational needs. As directed by a supervisor, performs other related and/or necessary tasks to achieve organizational and programmatic goals and objectives.
- Responsible for personal compliance with all applicable federal, state, local and center rules, regulations, protocols and procedures governing the practice of dental hygiene and the clinical provision of dental hygiene services as well as those relating to, but not limited to, personnel issues, workplace safety, public health and confidentiality.
- Working knowledge of dental equipment, tools, etc. Ability to establish and maintain effective professional relationships with fellow healthcare providers.
- Prolonged sitting or standing may be required.
- Physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with the practice of modern oral hygiene will be performed on a repetitive basis.
- Work under stressful conditions as well as irregular hours may be required.
- Frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations, radiation and other conditions common to a clinical environment may routinely be encountered.
- Bilingual, clinically fluent in Spanish preferred.
- Excellent interpersonal and written communications skills required.

This Community Health Center seeks a Dental Hygienist to provide excellent care to our patients.
This CHC was established in 1983. When the health department first opened, there was just one location and nine employees. The department has undergone incredible growth through the years and today, there are more than 140 employees working at seven different locations.
We provide preventative and basic dental services for any patient over the age of one. Services include: exams, cleanings, fluoride treatments, x-rays, fillings, extractions, crowns, partials and dentures.
Two dental clinic sites, one with 4 dental chairs and one with a 9 dental chairs.
