This Community Health Center seeks a Advanced Practice Registered Nurse - Family Nurse Practitioner to care for our patients in a manner that addresses physical, emotional, psychological and spiritual needs, recognizing these causes and others for poor health are often multi-factorial.

Our approach is to address the whole person. Our care team includes assessing our patients physical, spiritual, emotional, psychological and financial needs. We believe that by focusing on these causes we can greatly improve overall health and wellness.

Founded and developed in April 2000 by a group of family practitioners that got together to try and solve the lack of medical aid in the area, this CHC has consistently grown and now sees more than 22,000 patients of Chicago’s Southland communities, resulting in over 60,000 patient visits per year.
