Overview
Through our four sites in Little Village (South Lawndale), Marquette Park (Chicago Lawn), and Brighton Park, this Community Health Center provides bilingual, high quality primary care, behavioral health and wellness services to the community, regardless of immigration status, insurance status, or ability to pay to over 20,000 patients each year from communities across the Southwest Side.
Our commitment to offering care of the highest quality is made clear by our designation as a National Quality Leader by HRSA in 2016, 2017, 2019, and 2020, which ranks us among the top 3% of health centers in the nation and the top two in Illinois for our quality of care.
All full time providers and all clinical staff are bilingual.
Spanish speaking required.
Position is days, 40 hours per week.

PTO: Five weeks (26 days) for senior managers and providers.
Holidays: 10 per year, two of which (MLK Day and President’s Day) are floating.

up to $50 per month to quit smoking, maintain a healthy weight, get fit, and maintain healthy cholesterol and blood pressure levels.
CME/CEU: 5 days for medical providers plus $2000.
Licenses: State medical license, DEA registration.
Memberships: Hospital memberships paid. CME may be used for professional society membership
Eligibility for VISA Waiver programs
Subscription to Up-to-Date: Once per year.
Bonus: Providers Up to $8,000 per year for meeting quality goals.
