Overview
This is a professional, direct service position which utilizes an integrated care model of treatment. The Behavioral Health Provider is located within the primary care medical unit and provides behavioral health treatment to patients as part of the primary care health team. The Behavioral Health Provider works across the age range to deliver a broad array of warm hand-off and scheduled services including assessment, therapy (brief and long-term), crisis intervention, and coordination. The Behavioral Health Provider provides consultation services to primary care providers and collaborates with community organizations to ensure whole-person, patient-centered care. This position reports directly to the Lead Behavioral Health Provider.

Education Requirements
- Masters of Social Work, Counseling, Psychology or Human Service related field
- Active Illinois License (Licensed Clinical Social Worker, Licensed Clinical Professional Counselor, Licensed Marriage and Family Therapist, Licensed Clinical Psychologist)
- Experience in mental health clinical service delivery
- Previous work experience in a multicultural setting preferred
