Overview
This Community Health Center seeks a Family Medicine Physician to care for our patients in a manner that addresses physical, emotional, psychological and spiritual needs, recognizing these causes and others for poor health are often multi-factorial.
Our approach is to address the whole person. Our care team includes assessing our patients physical, spiritual, emotional, psychological and financial needs. We believe that by focusing on these causes we can greatly improve overall health and wellness.
Founded and developed in April 2000 by a group of family practitioners that got together to try and solve the lack of medical aid in the area, this CHC has consistently grown and now sees more than 22,000 patients of Chicago’s Southland communities, resulting in over 60,000 patient visits per year.
Essential Duties and Responsibilities
- Provide appropriate clinical services including wellness and lifestyle modifications.
- Performs physical examinations of new and existing patients while ordering additional diagnostic and screening tests as appropriate.
- Interprets examination findings and test results, and implements treatment plans
- Prepares and reviews case histories and obtains data through interviews
- Provides continuing, comprehensive health maintenance and medical care to the entire family unit, to include preventive medicine, behavioral sciences and community health
- Determines need for consultation and assists in medical care treatment provided at the direction of other specialists Instructs other health care providers and non-medical personnel in a variety of health related topics, including first-aid measures, basic and advances cardiac life support, and health care maintenance
- Participates in the instruction of medical students and residents who participate in a clinical rotation at FCHC
- Provides care consistent with the quality standards of the organization, participates in peer review activities, and works with administration to reduce risk and maintain patient safety.
- Performs other duties as assigned.

Job Requirements
Education, Training, and Experience
- Graduation from a college or university accredited by the American Medical Association with a Doctor of Medicine or accredited by the American Osteopathic Association with a Doctor of Osteopathy degree. If medical education occurred at a foreign medical school, the applicant must possess a current Educational Council Foreign Medical Graduate (ECFMG) Certificate. Successful completion of an approved program of residency training in a general care specialty.
Licenses or Certifications Required
- License to practice medicine in the State of Illinois plus current DEA registration
- A wellness certification is required which may be a Food As Medicine certification or a similar certification. A board certification is preferred including certification as a Culinary Medicine Specialist or Lifestyle Medicine certification through the American College of Lifestyle Medicine.
