Overview

The Wellness Nurse Practitioner is a member of the provider team performing a variety of direct patient services including assessment, diagnosis, treatment, evaluation, and monitoring of clinical management plans. They also promote wellness and provide health education to patients and their families; participate in planning programs and developing specific health policies and procedures.

Essential Duties and Responsibilities

  • Provide wellness services including nutrition, exercise counseling, and prevention services.
  • Provide direct patient care to a panel of patients across the lifecycle.
  • Assess, diagnose, prescribe, treat, evaluate, and monitor patients’ health status.
  • Maintain patient confidentiality in all matters and provide patient with needed information.
  • Attend meetings, seminars, workshops and conferences, as needed
  • Promote and maintain awareness of agency programs, mission statement, services, and activities.
  • Collaborate with physicians and other specialists as needed in patient care.
  • Make recommendations to the Medical Director and Clinical Management staff
  • Performs other duties as assigned.
  • Conducts daily team huddles.
  • Works closely with Care Team to ensure preventive, chronic and acute care needs are being met.
  • Participates in the Center’s ongoing quality improvement initiatives including meeting clinical standards, metrics for the UDS report, and health care plan goals as well as participating in peer review.

Job Requirements

Education, Training and Experience

  • The Nurse Practitioner must possess at least a MS degree from an accredited nursing school with a Family Nurse Practitioner Track focus.
  • Must have a certification as an Advance Practice Nurse that is recognized by a national certifying body as approved by the Illinois Department of Regulation Advance Practice Board and have a current and valid Illinois license as an Advance Practice Nurse with a DEA number and BLS certification.
  • Independent practice authority is preferred.
  • A wellness certification is also required which can include Food As Medicine certification, Certified Wellness Provider certification, or certification as a Culinary Medicine Specialist.
