Overview
This CHC serves the Rock River Valley area with quality primary health care for all people in need across our eight locations. Founded in 1972, this Community Health Center is committed to providing a collaborative healthcare system that is patient-centered in all aspects, affordable and accessible, and high quality with dignity, compassion, and cultural sensitivity.
The Women’s Health Nurse Practitioner provides primary care services to children and/or adults as part of the provider team at the FQHC collaborating with licensed MD/DO physicians. Care may be provided to any program or site of the FQHC. The Women’s Health Nurse Practitioner will collaborate and consult when necessary with patients, families, FQHC support staff, other midlevel providers, OBGYN physicians as well as internal and external consultants in order to provide the appropriate high level of care. This position reports to the Women’s Health Services Department Chair.

EDUCATION REQUIREMENTS
- Graduate of United States accredited Nurse Practitioner Program, new graduates must pass licensing exam
- Unrestricted Illinois Nurse Practitioner license, Illinois Controlled Substance License, Federal DEA, Illinois RN license
- Current CPR Certification (responsible for maintaining certification)
- Spanish fluency highly desired

COMPENSATION The salary range for this position is $114,441-$146,484. Actual compensation will be determined based on factors such as relevant experience, qualifications, and internal equity.
