The Staff Physician is responsible for comprehensive care to patients under the direction of the CMO or Medical Director per medical protocols. This position will collaborate with Nurse Practitioners, PA's and Teaching Assistants (TA). M.D. or D.O. from an accredited Medical School, Board Certified. Completion of a Residency Program in specialty. Current and valid Illinois license as a physician and surgeon. Current and valid DEA (Federal) and Controlled Substance Registration Certificate (State of Illinois).

This Community Health Center seeks a caring and dedicated OB/GYN physician to provide sensitive and approachable care with attention to cancer prevention, family planning, and ensuring prepared and healthy new moms and babies.

Since 2004, this CHC has provided bilingual, high quality primary care, behavioral health and wellness services to the community, regardless of immigration status, insurance status, or ability to pay. Our five sites in Brighton Park, Gage Park, Little Village, Marquette Park, and South Lawndale deliver care to over 35,000 patients each year from communities across the Southwest Side.

Location: Niles, IL - Specialty: Pediatric, General

Under the supervision of Chief Medical Officer, provides and manages direct health care for a specified patient population.

DUTIES AND RESPONSIBILITIES:

  • Examines, diagnoses, treats and/or refers patients assigned to physician's panel, prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Maintains a problem oriented health record for each patient seen in accordance with the Center's established policies and procedures to include providing appropriate and legible documentation.
  • Assures continuity of care by providing comprehensive medical services on a timely basis as per Clinic schedule within the Center's scope of practice and the provider's training.
  • Refers clients for appropriate specialty care services, lab and x-ray and any other ancillary services that are appropriate for patient's management and care.
  • Reviews all lab and x-ray reports for patients under his/her care on a timely basis and makes medical management decisions appropriately.
  • Maintains appropriate productivity standards of specialty as established by the Center; maintains appropriate and efficient patient flow and minimize schedule changes and absences.
  • Provides excellent internal/external customer service via telephone, fax or face-to-face contact to assist patients with their health care needs.
  • Trains, mentors and supervises students and clinical support staff, as appropriate.
  • Takes call after hours and on weekends on a rotation basis with other providers and responds appropriately and on a timely manner to after hour calls.
  • Follows established clinic policies, procedures, and objectives; continuous quality improvements objectives and safety, environmental, and/or infection control standards.
  • Participates in specified health promotion, education and/or prevention programs as needed i.e. diabetes collaborative/health fairs etc.; attends and participates in clinic meetings / departmental meetings and other clinic functions as required by the Center.

POSITION SUMMARY:

The Pediatrician provides primary care services to children as part of the provider team.

ESSENTIAL FUNCTIONS: This will include but is not limited to:

  1. Accountable for demonstrating the qualities outlined in the “Customer Service Standards of Excellence” program when interacting with patients, their families and friends, and fellow employees.
  2. Understand and comply with the Mission Statement and Vision Statement of the FQHC.
  3. Provide care and establish within the medical record problem lists, medication lists and records deemed necessary to longitudinally follow the patient. Care may be provided to any program or site of the FQHC.
  4. Collaborate and consult, when necessary, with patients, families, support staff, midlevel providers, internal and external consultants in order to provide the appropriate high-level care.
  5. Assume responsibility of patient continuity of care.
  6. Participate in health promotion and disease prevention activities for all patients.
  7. Collaborate and mentor mid-level providers.
  8. Participate in quality improvement and assurance activities.
  9. Attend mandatory staff meetings and in-services.
  10. Complete records to the detail deemed necessary by regulatory agencies and in a timely fashion.
  11. Attend and participate in FQHC activities.
  12. Embrace and support health care change.
  13. Aid in the construction of a collaborative work environment.
  14. Maintains an adequate amount of continued medical education credits to meet licensure, hospital medical staff, health plan and sill sets necessary to provide a high level of care to patients.
  15. Other duties as assigned by the CEO or CMO or his/her designee to meet the goal of providing excellent health care.

POSITION SUMMARY: Under direction of the Medical Director, provides medical services in the community health center.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Exams, diagnoses, and treats diseases and injuries of infants, children, adolescents and young adults birth to age 17.
  2. Examines patients and determines need for x-ray examinations and clinical laboratory tests.
  3. Interprets examinations findings and test results, and implements pediatric treatment plans.
  4. Prepares and reviews case histories and obtains data through interviews.
  5. Supports health promotion and disease prevention activities to enable each child to reach full potential.
  6. Monitors physical and psychosocial growth and development.
  7. Conducts age-appropriate screening.
  8. Provides advice and education for patients and parents regarding appropriate preparation for predictable developmental challenges.
  9. Determines need for consultation and assists in medical care treatment provided at the direction of other specialists.
  10. May instruct medical students and /or residents in procedures for diagnosis and treatment of diseases and injuries of infants and children.
  11. Participates in the instruction of medical students and residents who participate in a clinical rotation at FCHC.
  12. Provides care consistent with the quality standards of the organization, participates in peer review activities, and works with administration to reduce risk and maintain patient safety.
  13. Performs other duties as assigned.

Duties and Responsibilities

  • Practice of medicine within the scope of license, training, credentialing and in accordance with the current standards of care and quality criteria adopted by the FQHC.
  • See pediatric patients in FQHC clinics. This is a broad-spectrum pediatric practice.  
  • Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
  • Order, perform and interpret tests and analyze records, reports, and examination information to diagnose patients’ condition.
  • Complete charting on an electronic medical record and required paperwork in a timely manner. Maintain confidential patient information.
  • Newborn nursery rounding of pediatric patients at Ascension Saints Mary and Elizabeth Medical Centers - occasionally midday throughout week and one Saturday every 4 weeks.
  • Participation in teaching activities with the Saints Mary and Elizabeth Family Medicine Residency Program including precepting residents and attending faculty meetings as needed.
  • Participate in the development/review of organizational policies and quality improvement initiatives.

Pediatrician - Position Summary:

Provide the full scope of services which fall under provider’s field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care, and electronic health record (EHR) maintenance, while following guidelines contained within the quality management, risk management, infection control, customer service and safety programs.

Clinical and Operational Responsibilities

General Clinical 

  • Obtains health history and perform physical examinations. 
  • Diagnoses, treats, counsels, and coordinate care for each patient. 
  • Reviews and update problem list and longitudinal plan of care at every visit. 
  • Meets documentation standards and completes all progress notes within 24 hours following the patient visit. 
  • Fulfills on-call responsibilities in accordance with the schedule developed and responds to calls within 20 minutes. 
  • Prescribe medication in accordance with Illinois statute and professional practice guidelines. 
  • Performs all duties in conformity with the quality standards and clinical guideline.
  • Meets Meaningful Use standards which is defined as using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities, engage patients and families in their health care, improve care coordination, improve population and public health; all the while maintaining privacy and security. 
  • Manages EHR In-Basket, transfers In-Basket responsibilities to colleagues when appropriate, and responds to messages (e.g. lab results, refills, patient calls, patient messages, co-sign orders, co-sign charts, and open charts) within 72 hours

Patient Support and Education 

  • Provides culturally competent care 
  • Recognizes patients’ families as an integral part of patients’ care teams. 
  • Ensures that patients’ and families’ needs are met, and clinic policy is followed through. 
  • Encourage patients and families to voice questions and/or concerns. 
  • Works with patients and their families as appropriate to encourage care self-management 
  • Conducts patient education activities, including pre and post instruction exams and procedures, instructions related to illness and treatment, and reinforcement of provider’s follow-up instructions 
  • Utilizes printed health education materials as appropriate and document all instructions given; while giving consideration to an individual varying level of understanding and adjusts level of presentation to ensure the patient is knowledgeable of medical condition and plan of care 
  • Works with care team and case management to provide for coordinated delivery of case management services. 
  • Ensures patients presenting with greater acuity than what is clinically appropriate within the health center are transferred appropriately 
  • Meets intensity of service and/or severity of illness admission criteria
