Making a difference in the lives of vulnerable patients, this FQHC is a community health center in Champaign, Illinois. We are seeking an experienced (2 years minimum) Advanced Practice Clinician to provide care across the lifespan. Certification as a Physician Assistant and completion of required Physician Assistant Program or MSN or higher degree and certification as a Nurse Practitioner. Must be currently licensed to practice in the State of Illinois or eligible for license in Illinois. DEA required.

As a part of a multi-disciplinary team of healthcare providers, the Family Nurse Practitioner will work collaboratively across all departments to remove barriers for accessing healthcare, incorporating standard medical practices and health center policies into their clinical practice to provide quality care commensurate with training and licensure. Care delivered includes comprehensive health wellness, preventive and medical care to multicultural, multilingual patients

This Community Health Center began as a volunteer-staffed health center, created in the 1960's to address the issue of high infant mortality in the African-American community. It has since transformed into a health center with staff physicians dedicated to providing primary health care to the underserved in our community. In 2006, the CHC moved to its current location on Bloomington Road in Champaign. In 2011, a Dental Center opened in the same location.

This organization provides over 40,000 patient visits and cares for over 11,500 patients annually. The health center is a family practice clinic and delivers integrated care, with on-site mental health counselors, psychiatrists, and a dental clinic. Patients are supported for social service needs. The health center has a great referring relationship with providers in the community, which provides our patients great access to specialty care.

Nurse Practitioner, Family

  • Assesses and interprets medical and psycho-social histories; performs physical examinations (including gynecological and breast examination as appropriate); identifies problems; formulates a plan of treatment, provides education and referral (as indicated); and evaluates effectiveness of treatment, education and/or referral.
  • Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral source when indicated.
  • Orders, may perform and interprets the results of laboratory tests as part of the physical examination and health management. Reports communicable disease in a timely manner to the appropriate Health Dept. per applicable procedures and guidelines.
  • Conception and contraception management per applicable protocols and state and federal family planning program guidelines for women presenting for family planning services. This partially includes as indicated: confidential family planning services for adolescents; pregnancy testing and options counseling; provision of emergency contraception; and consideration of all FDA approved contraceptive methods for women seeking contraception.
  • Conducts health teaching and counseling during the physical exam, as appropriate, enabling the client to make informed choices about health and treatment options.
  • Completes client clinical record documentation in a timely manner specific to the clinical services provided, according to applicable policies and procedures.
  • Applies exposure control procedures as described in the FQHC's Infection control policy and Procedure Manual.
  • Understands the medical emergency procedures and is familiar with the location of the emergency kit(s)/supplies in all clinic settings.

About the Role:

As an APRN you will serve as a primary care provider responsible for delivering direct medical care to patients of all ages in a team-based setting. You will collaborate with physicians, nurses, behavioral health providers, and administrative staff to provide high-quality, coordinated care. This role also supports the clinical leadership and growth of the practice through quality improvement efforts and mentoring of clinical team members.

Key Responsibilities:

  • Perform comprehensive patient assessments, physical exams, and health screenings
  • Diagnose acute and chronic conditions and develop appropriate treatment plans
  • Prescribe medications, including psychotropics, and manage medication therapies

POSITION SUMMARY:

ESSENTIAL FUNCTIONS: This will include but is not limited to:

This Community Health Center seeks a Psychiatric Nurse Practitioner to provide behavioral health care services and medication management to patients in all life cycles.

Under collaborative agreement with a VNA Health Center physician, provides comprehensive medical evaluation, education and management for clients throughout the life cycle that present for routine preventive, acute and chronic disease health care services at the VNA Health Center clinic sites.

Job Summary
Responsible to the Chief Medical Officer (CMO). A management-level position that provides supervision, professional assistance, technical advice/training, and is responsible for supporting quality care in Primary Care, Behavioral Health, and Women’s Health at all Health Center locations. Scheduled hours are two days of the week 8am – 4pm, three days of the week 9am – 5pm, and a rotating Saturday from 9am – 3pm every 4-6 weeks.

Job Summary

Under the direct supervision of the Clinic Manager, the applicant will provide general nursing care and leadership while working collaboratively with physicians and other multidisciplinary team members. They will educate individuals and families in health promotion and disease prevention and provide instruction in general health care and treatment to assigned patients. The staff nurse will promote and restore patients’ health as they provide, delegate, evaluate, and coordinate comprehensive professional nursing care using the nursing process for patients of all ages, genders, cultures, and backgrounds while providing physical and psycho-support to patients, friends, and families. Schedule calls for Monday through Friday, 9:00 am-5:00 pm, and travel to satellite locations to provide coverage as needed.
