Description:

Description:

Under collaborative agreement with a VNA Health Center physician, provides comprehensive medical evaluation, education and management for clients throughout the life cycle that present for routine preventive, acute and chronic disease health care services at the VNA Health Center clinic sites. 

Primary Functions:     

  1. Assesses and interprets medical and psycho-social histories; performs physical examinations (including gynecological and breast examination as appropriate); identifies problems; formulates a plan of treatment, provides education and referral (as indicated); and evaluates effectiveness of treatment, education and/or referral.
  2. Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral source when indicated.
  3. Orders may perform and interprets the results of laboratory tests as part of the physical examination and health management. Reports communicable disease in a timely manner to the appropriate Health Dept. per applicable procedures and guidelines.
  4. Conception and contraception management per applicable protocols and state and federal family planning program guidelines for women presenting for family planning services. This partially includes as indicated: confidential family planning services for adolescents; pregnancy testing and options counseling; provision of emergency contraception; and consideration of all FDA approved contraceptive methods for women seeking contraception.
  5. Conducts health teaching and counseling during the physical exam, as appropriate, enabling the client to make informed choices about health and treatment options.
  6. Completes client clinical record documentation in a timely manner specific to the clinical services provided, according to applicable policies and procedures.
  7. Applies exposure control procedures as described in the Infection control policy and Procedure Manual.
  8. Understands the medical emergency procedures and is familiar with the location of the emergency kit(s)/supplies in all clinic settings.
  9. Maintains professionalism and adheres to client confidentiality in all VNA Health Center practices.
  10. Participates in quality improvement activities, including, but not limited to periodic clinic observation of clinical/lab skills by the medical director or designee; chart audits; outcome studies; client satisfaction surveys; clinic flow analysis; proficiency testing for CLIA; and quality management reporting.

Summary:

The Obstetrician/Gynecologist provides for the primary health care needs of women patients according to the philosophy and goals of the FQHC and in collaboration with the Nurse-Midwifery service of the medical center.. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Essential Duties and Responsibilities: 

  • Provides patient care in a specified number of clinic sessions per week
  • Provides out-patient clinic consultation and in-patient backup for the Nurse-midwifery service, for a specified amount of time each month
  • Admits patients to the hospital as necessary and serves as primary attending physician
  • Provides comprehensive integrated patient care in cooperation with other programs at the FQHC (i.e., WIC, prenatal classes, nutrition counseling, case management, health education)
  • Works cooperatively with other physicians for cross-coverage of patients, both clinic and in-patient when appropriate
  • Participates in Quality Assurance activities
  • Participates in data collection of the maternity service
  • Co-signs in-patient midwife charts in medical records while covering as backup physician

POSITION SUMMARY:

The Obstetric and Gynecology physician provides obstetric and gynecological services to women as part of the provider team at the FQHC. 

ESSENTIAL FUNCTIONS: This will include but is not limited to:

This Community Health Center seeks an OB/GYN Physician who will provide high quality medical care to our patients in a culturally-sensitive and linguistically diverse environment.

Our patient population is an incredibly diverse group of people including many immigrants and refugees. We serve people from 55 different countries, and in more than 25 different languages, including multiple Asian, African and European languages. All of our highly-trained providers are bilingual and speak a variety of languages which allows patients access to a provider who can speak their language.

This CHC was incorporated in 2001 as an independent not-for-profit to increase access to primary care for underserved communities on Chicago’s north side. The organization grew from its humble beginnings and opened its doors as a full-time Federally Qualified Health Center (FQHC) on March 1, 2004 in the West Ridge neighborhood

The OB/GYN Physician will:

  • Provide and manage direct patient care, including physical examinations, evaluations, assessments, diagnoses, and treatment for a specified patient population.
  • Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Provide prenatal and perinatal care per evidence-based guidelines.
  • Perform cesarean sections or other surgical procedures as needed to preserve patients' health and deliver babies safely.
  • Maintain delivery and cesarean section privileges at affiliated hospital(s).
  • Refer patients to specialists and to relevant patient care components as appropriate.

This Community Health Center seeks an OB/GYN Physician to provide high-quality, patient-centered care to ensure healthy pregnancies for mothers and improved birth outcomes for babies.  The OB/GYN will provide the full scope of services which fall under provider’s field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care, and electronic health record (EHR) maintenance, while following guidelines contained within the quality management, risk management, infection control, customer service and safety programs for the FQHC.

Clinical and Operational Responsibilities

General Clinical 

  • Obtains health history and perform physical examinations. 
  • Diagnoses, treats, counsels, and coordinate care for each patient. 
  • Reviews and update problem list and longitudinal plan of care at every visit. 
  • Meets documentation standards and completes all progress notes within 24 hours following the patient visit. 
  • Fulfills on-call responsibilities in accordance with the schedule developed and responds to calls within 20 minutes. 
  • Prescribe medication in accordance with Illinois statute and professional practice guidelines. 
  • Performs all duties in conformity with the quality standards and clinical guideline established. 
  • Meets Meaningful Use standards which is defined as using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities, engage patients and families in their health care, improve care coordination, improve population and public health; all the while maintaining privacy and security. 
  • Manages EHR In-Basket, transfers In-Basket responsibilities to colleagues when appropriate, and responds to messages (e.g. lab results, refills, patient calls, patient messages, co-sign orders, co-sign charts, and open charts) within 72 hours

Patient Support and Education 

  • Provides culturally competent care 
  • Recognizes patients’ families as an integral part of patients’ care teams. 
  • Ensures that patients’ and families’ needs are met, and clinic policy is followed through. 
  • Encourage patients and families to voice questions and/or concerns. 
  • Works with patients and their families as appropriate to encourage care self-management 
  • Conducts patient education activities, including pre and post instruction exams and procedures, instructions related to illness and treatment, and reinforcement of provider’s follow-up instructions 
  • Utilizes printed health education materials as appropriate and document all instructions given; while giving consideration to an individual varying level of understanding and adjusts level of presentation to ensure the patient is knowledgeable of medical condition and plan of care 
  • Works with care team and case management to provide for coordinated delivery of case management services. 
  • Ensures patients presenting with greater acuity than what is clinically appropriate within the health center are transferred appropriately 
  • Meets intensity of service and/or severity of illness admission criteria
