This Community Health Center seeks a Dental Hygienist to provide excellent care to our patients.
This CHC was established in 1983. When the health department first opened, there was just one location and nine employees. The department has undergone incredible growth through the years and today, there are more than 140 employees working at seven different locations.
We provide preventative and basic dental services for any patient over the age of one. Services include: exams, cleanings, fluoride treatments, x-rays, fillings, extractions, crowns, partials and dentures.
Two dental clinic sites, one with 4 dental chairs and one with a 9 dental chairs.

Position Summary
The Dentist (Clinical Hourly) will provide clinical dental care within the scope of license and credentials at the FQHC clinics in conformance with approved clinical protocols and guidelines.
Duties & Responsibilities
Patient Diagnostics and Treatment
- Examine patients to determine nature of condition, utilizing x-rays, dental instruments, and other diagnostic tools/procedures.
- Diagnose and treat diseases, injuries and malformations of teeth, gums, and related oral structures.
- Clean, fill, extract, and replace teeth, using rotary, hand instruments, dental appliances, medications, and surgical implements.
Preventative Treatment and Patient Education
- Provide preventive dental services to patients, including fluoride applications, and other sealants to teeth.
- Provide continuous education to patients with respect to oral and dental hygiene.
Administration
- Record patient-dentist transactions as they occur accurately and completely reflecting the nature of the contact, condition of the patient, and the care or treatment provided.
- Complete referrals, data collections instruments, and other records as required.
- Assist with the design, development, and implementation of appropriate Dental Department policies and protocols in compliance with the most current accepted professional standards.
- Assist with continuing education, on-the-job training, and orientation of community health center staff as requested.

This Community Health Center is seeking a Dentist to provide general dental services to children and adults as part of the provider team. This CHC serves the Rock River Valley area with quality primary health care for all people in need across our eight locations. ESSENTIAL FUNCTIONS:

We are seeking a skilled and compassionate dentist to join our team and provide exceptional dental care to patients our surrounding communities. The Dentist will work under the guidance of the Dental Director to deliver high-quality dental services to our patients while adhering to CFHC policies and industry best practices. Responsibilities:
- Examines patients requesting care, diagnose their dental/oral conditions, prescribes and carries out, or directs others in carrying out, appropriate dental treatment within the scope of services credentialed, or refers individuals for specialty consultation or treatment according to approved clinical protocols and guidelines.
- Records in a timely manner patient-dentist transactions in the patient’s dental record so that the dental record accurately and completely reflects the nature of the contact, the condition of the patient and the care or treatment provided. In addition the Dentist shall complete referrals, data collection and dental laboratory work orders and other records as shall be required by the CFHC.
- Educate patients in the nature of oral health related conditions and in the general promotion of oral health disease prevention.
- Participates in 100 % peer review, chart audit and Quality Improvement programs.
- Maintain an environment of cleanliness and practice standard infection procedures.
- Assists in the provision of continuing education, on-the-job training, and the orientation and supervision of dental support staff as needed.

The Dentist provides comprehensive clinical dental services within a community health center setting. The position is responsible for the diagnosis, prevention, and treatment of oral diseases and conditions affecting patients of all ages. The Dentist supports the mission and values of the FQHC by delivering high-quality, ethical, and patient-centered care while contributing to the overall health of the community.
Essential Duties and Responsibilities
- Administers local anesthesia routinely and prescribes analgesics and other medications when clinically indicated.
- Exposes and interprets intra-oral and extra-oral radiographs to establish accurate diagnoses and develop treatment plans.
- Performs oral surgical procedures including extractions and removal of foreign bodies or anomalies.
- Provides restorative and preventive dental services to eliminate infection and restore function.
- Identifies and treats local dental diseases and oral manifestations of systemic conditions. Initiates appropriate consultations or referrals when conditions exceed scope of practice.

This FQHC is seeking a compassionate Full-Time Dentist to deliver high-quality dental care to individuals experiencing homelessness and participants in the Ryan White Program.
Essential Duties and Responsibilities:
- Facilitates entry of the client into the oral health care system by assessing the primary oral health care needs, including eliciting comprehensive health histories, performing oral examinations, order and/or performing diagnostic tests, analyzing collected data to determine client health status and formulating a problem list (diagnosis).
- Provides coordinated, continuous, and culturally competent oral health care to clients.
- Prescribes a variety of treatment modalities made available to the provider through Health Center’s resources to meet preventative oral health and disease needs of participants.
- Collaborates with physicians and dental auxiliaries to assure that oral health instruction and counseling is provided.
- Manages and evaluates the plan of care, process, and outcome, and then modifies plan as needed.
- Coordinates services in conjunction with other team members to meet client's oral health care needs, and to ensure the continuity of care.
- Collaborate with other multi-disciplinary professionals and community agencies to provide high quality care, including referral to appropriate dental specialists, if indicated.
- Providers are expected to continue follow up with those referred to dental specialist to monitor status of such referrals in the continuum of care for that individual to promote completion of treatment or completion of alternatives to care.
- Adheres to collaborative principles of respect and joint decision making.
- Participates in quality improvement activities though protocol development, staff training and collaborative peer review.
- Assures access and continuity of care by providing call for after-hours emergency oral health coverage and follow-up at the primary treatment facility or at the client residence if necessary and feasible.
- Assumes the responsibility for professional development and contributes to the professional growth of others by:
- Practices within legal and ethical guidelines established by the Dental Practice Act and the Dentist Code of Ethics. Adheres to professional standards as outlined by governmental bodies, the American Dental Association (ADA), private funding sources, agency plans/policies and Unit guidelines.
- Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understand and continues to adhere to such standards.
- Initiates requests for assistance or guidance from Supervisor to address new issues or complex concepts affecting adherence to professional standards.
- Prepares the appropriate program reports to fulfill funding requirements as assigned.
- Documents comprehensive, accurate, and continual data on clients' records and program reports according to agency standards.

- Educate patients on oral hygiene, preventative care, and treatment options to promote long-term oral health
- Maintain accurate, timely, and compliant documentation in the electronic health record (EHR)
- Collaborate with medical providers and care teams on integrated initiatives and oral-systemic health management
- Supervise and collaborate with dental hygienists, dental assistants, and support staff to ensure efficient clinic operations
- Follow infection control, sterilization, and OSHA safety protocols at all times
- Attends and participates in all staff meetings
