Location: Champaign, IL - Specialty: Family Medicine, General

As a part of a multi-disciplinary team of healthcare providers, the Family Medicine Physician will work collaboratively across all departments to remove barriers for accessing healthcare and improve the health of the community through treatment, prevention, and education. Care delivered includes comprehensive health wellness, preventive and medical care to multicultural, multilingual patients

This Community Health Center has deep roots in Champaign's underserved communities and serves a diverse population. It began as a volunteer-staffed health center, created in the 1960's to address the issue of high infant mortality in the African-American community. It has since transformed into a health center with staff physicians dedicated to providing primary health care to the underserved in our community. 

This organization provides over 40,000 patient visits and cares for over 11,500 patients annually. Approximately 30% of patients are Spanish speaking. The health center is a family practice clinic and delivers integrated care, with on-site mental health counselors, psychiatrists, and a dental clinic. Patients are supported for social service needs. The health center has a great referring relationship with providers in the community, which provides our patients great access to specialty care.

Position is 40 hours/week (36 direct patient care).

Our patients and staff are at the heart of everything we do. For over 100 years, this FQHC look-alike has provided health care services to medically underserved and vulnerable Chicagoans. Today, we provide pediatric health, adult health, pediatric and family dentistry, prenatal Care, including Centering Pregnancy™, care coordination, counseling, behavioral health, speech therapy, occupational therapy, social skills groups, optometry.  This model helps ensure access to comprehensive services for medically underserved families in Chicago. 

  • Manage primary, acute and chronic health problems affecting women through comprehensive history and physical examination, assessment and diagnostics in outpatient setting
  • Order, perform and interpret diagnostic and therapeutic tests and procedures
  • Management of full spectrum of contraception
  • Prescribe pharmacologic and non –pharmacologic interventions
  • Collaborate and coordinate with the primary care team to deliver evidence-based care and to establish best practices in clinical care

Location: Decatur, IL - Specialty: Family Medicine, General

This academically affiliated Federally Qualified Health Center is seeking qualified applicants to fill a faculty position at the Assistant or Associate Professor level. This position is a faculty position within the Family Medicine Residency Program within the Center for Family Medicine in Decatur. The position includes teaching residents, medical students and physician assistant students as well as direct patient care responsibilities in the inpatient and outpatient settings. Research opportunities also exist. Two additional positions are available, one that is only outpatient and one position doing only inpatient work. 

Preference will be given to applicants who practice full scope family medicine. The practice of Obstetrics is desirable but not required. Essential Duties and Responsibilities

  • Provide appropriate clinical services including wellness and lifestyle modifications.
  • Performs physical examinations of new and existing patients while ordering additional diagnostic and screening tests as appropriate.
  • Interprets examination findings and test results, and implements treatment plans
  • Prepares and reviews case histories and obtains data through interviews
  • Provides continuing, comprehensive health maintenance and medical care to the entire family unit, to include preventive medicine, behavioral sciences and community health
  • Determines need for consultation and assists in medical care treatment provided at the direction of other specialists Instructs other health care providers and non-medical personnel in a variety of health related topics, including first-aid measures, basic and advances cardiac life support, and health care maintenance
  • Participates in the instruction of medical students and residents who participate in a clinical rotation at FCHC
  • Provides care consistent with the quality standards of the organization, participates in peer review activities, and works with administration to reduce risk and maintain patient safety.
  • Performs other duties as assigned.
