This Community Health Center provides quality, cost-effective healthcare to minority communities, the uninsured, and underinsured of Pilsen Little Village, including Primary Care, Behavioral Health, Dentistry, OB/GYN and Midwifery, Pediatrics, Pharmacy, and In-House Lab Services. 

The Family Nurse Practitioner will:

  • diagnose, treat and help prevent mental disorders in a community health center setting

This Community Health Center seeks a Spanish-speaking Nurse Practitioner to provide primary care services to patients in all life cycles in a medical home model. The Nurse Practitioner will prevent disease, help patients learn about healthy choices, and treat disease and illness.

This Community Health Center (CHC) primarily serves the Chicago communities of Austin, Belmont-Cragin, Dunning, East Garfield Park, Hermosa, Humboldt Park, Logan Square, Montclare, New City, North Lawndale, Portage Park, West Garfield Park, West Town, and the near west suburbs of Berwyn, Cicero, Maywood, Melrose Park, and Oak Park.

This CHC began in 1980 as the Parent Child Center, a three-room clinic at West Suburban Medical Center that offered prenatal, postpartum, and infant care for underserved residents of Chicago’s Austin community. In 1992, this CHC attained the status of a Federally Qualified Health Center (FQHC) look-alike in 1994 and received full FQHC recognition in 2002. Since then, we have grown to encompass thirteen health centers, serving West Side Chicago and the near west suburbs. 

Most of our sites are family health centers, offering a full range of services for all ages. We also operate a medical clinic inside a respite shelter for people who are homeless, a school-based health center in Belmont-Cragin, a community farm in Austin, and northern Illinois’ only freestanding birth center, in Berwyn.

Position Description

The Physician Assistant or Nurse Practitioner (PA-NP) is responsible for managing total patient care in an outpatient setting. This role requires clinical expertise, strong diagnostic and treatment skills, and the ability to work collaboratively within a multidisciplinary team to provide high-quality healthcare services to the community.

Scope of Work Responsibilities

  • Elicit and record accurate health histories.
  • Perform complete physical assessments of appropriate body systems utilizing mechanical methods and instruments including, but not limited to, otoscope, ophthalmoscope, anoscope, stethoscope, and speculum.
  • Interpret laboratory and X-ray results. Assess social, cultural, and family history.
  • Utilize written medical protocols and/or consult with collaborating physician to plan medical and nursing therapies.
  • Differentiates between normal and abnormal findings; initiates treatment and/or refers patients to specialists as required.
  • Conduct screening for communicable diseases; provide follow-up assessment, documentation, evaluation, and reporting as needed.
  • Train, supervise, and teach other staff members directly assisting PA or NP.
  • Provide community education as requested.
  • Perform short-term patient counseling and/or refer to appropriate counseling services.
  • Participate in continuing education and in-service programs.
  • Perform related duties as assigned.

  • Participate in FQHC committees when requested.
  • Provide after-hours call per shared schedule and document calls in patient records.

Site Work Location Each employee has a primary work site(s) to which he/she is assigned. The FQHC operates multiple sites; therefore, employees may be asked to work at other sites as needed. Administrative employees and contracted service providers may work from home as agreed upon in their contract.

Work Environment

This position is performed in a health center setting with moderate noise levels in a moderately paced environment. Frequent patient or client contact occurs both in person and via telephone. The employee will collaborate with other FQHC departments to deliver optimal service and meet health center goals.

POSITION SUMMARY: The Nurse Practitioner provides primary care services to children and/or adults as part of the provider team collaborating with licensed MD/DO physicians.

ESSENTIAL FUNCTIONS: This will include but is not limited to:

  1. Accountable for demonstrating the qualities outlined in the “Customer Service Standards of Excellence” program when interacting with patients, their families and friends, and fellow employees.
  2. Understand and comply with the Mission Statement and Vision Statement of the FQHC.
  3. Provide care and establish within the medical record problem lists, medication lists and records deemed necessary to longitudinally follow the patient.

  5. Deliver medical care under the direction of a primary supervising physician and secondary supervisory physician staff. Use appropriate protocols, guidelines, and agreed upon references and clinical care understandings. Care may be provided at any program or site at the FQHC.
  6. Assume responsibility of patient continuity of care.
  7. Participate in health promotion and disease prevention activities for all patients.
  8. Participate in quality improvement and assurance activities.
  9. Attend mandatory staff meetings and in-services.
  10. Complete records to the detail deemed necessary by regulatory agencies and in a timely fashion.

This FQHC opened its doors in 1989 as a bilingual, bicultural, nonprofit community health center, located in the heart of the Pilsen community. This organization operates as a FQHC through outpatient clinics with one urgent care location, three traditional outpatient clinics and three school-based clinics. Their services range from Adult & Family Medicine, Behavioral & Mental Health, Dentistry, Diabetes, Family Planning, OB/GYN & Midwifery, Pediatrics, Pharmacy, and Walk In Wellness Center.

The Nurse Practitioner provides primary health care to specified patient populations of the FQHC.  The Nurse Practitioner utilizes theories of psychosocial, developmental, educational and nursing sciences to facilitate the delivery of this health care. The Nurse Practitioner works inter-dependently with and independently of the multi-disciplinary team.

The Family Nurse Practitioner or Physician Assistant works under the direct supervision of the Chief Medical/Population Health Officer, who is also the collaborating physician. FNP/PA completes all the duties associated with their clinical license and manages patient care in accordance with the professional guidelines of the FQHC. This position is unique as it is a collaborative position between the FQHC in LaGrange and in Summit. The objective is to optimize health for those adults experiencing housing insecurity by improving access to primary care FNP/PA will spend 50% of their time providing medical assessment and care in a clinical location at the Summit, which is a dedicated location for individuals experiencing homelessness, and 50% at the FQHC in La Grange.  

Essential Duties and Responsibilities:

Patient Care

  • Provides general medical care and treatment to patients under the direction of a physician and as dictated by their Illinois Clinical License and PCH policies and procedures
  • Provides primary patient health assessment.
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physicians to diagnose and treat acute, chronic, and long-term health issues of patients.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
  • Prescribes or recommends medication or other forms of medical treatment such as therapeutic procedures.

Case Management

  • May refer patients to physicians for consultation or to specialized health resources for treatment.
  • Coordinates patient treatment, management and planning with the clinical staff and physician to provide continuity of care, including assistance with referrals to other agencies and health care providers.
  • Submits health care plan and goals of individual patients for periodic review and evaluation by physician.
  • Provides telephone support to patients, physicians hospitals, pharmacies, and other health care providers.

Patient Records

  • Responsible for documentation of patient care; records physical findings, and formulates plan and prognosis, based on patient’s condition.
  • Maintains patient information and confidentiality in accordance with applicable laws and PCH policies.

Patient Education

  • Provides education to patients and/or their families about preventive care, medical issues and the use of prescribed medication and treatments.
